Hypertension and renal disease in diabetes are areas where developments rapidly occur constantly and, therefore, there is a clear need for a new edition, although it is only three years since the last. During these three years, there have been many important new publications and numerous new trials have been conducted. Increasing evidence is now put on screening which means identifying patients early by measuring microalbuminuria. Microalbuminuria is a unique predictor of worsening renal disease, cardiovascular diseases as well as early mortality. An increasing number of studies documents that treatment improves prognosis.
Unfortunately, the number of end-stage renal disease related to type 2 diabetes is increasing, but there seems to be a decline, at least in some countries of patients with type 1 diabetes; it seems that also the age at which patients enter advanced renal disease programmes has increased, which is probably related to greatly improved medical care in general.
